CURRICULUM PROPOSAL FORM #6

 OTHER CURRICULAR ACTION

This curricular proposal form is for all curricular action not covered by forms 1-5, with the exception of special courses and contuiation of inactive courses. It goes through the usual curricular committee process (see Matrix).

Effective Term
Indicate the first academic term the new action is to be in effect.
Remember to take into consideration the time needed for approval/recording.

Description of Action
Give a breif description of the Action.  Possible usage would include:

  1. Changing the Proficiency Requirement
  2. Changing the General Education Requirement
  3. Changing the Diversity Requirement
  4. Changing the Licensure Requirements
  5. Changing Number of Credits Required for Graduation

NOTE:  If the type of change is on the matrix, use the comments in the dictionary as a guideline for format.  If not, contact the Associate Vice Chancellor's Office for assistance.

Sponsor(s)
Enter the name of the individual(s) writing the proposal.

Department(s)
Enter the name of the department(s) responsible for the course.

College(s)
Select the name of the college(s) responsible for the new course.  Select "Interdisciplinary" if more than one college is responsible for the course.

Proposal
Describe the action in detail

Justification
Within the rationale, include an explanation of the relationship of the proposed changes to your program assessment objectives and a budgetary impact statement if appropriate.
